Betaine HCl is a supplement form of hydrochloric acid, taken on the theory that some reflux is caused by insufficient stomach acid (hypochlorhydria) rather than excess acid — the opposite mechanism that standard PPI treatment addresses.

Who this theory applies to, if anyone

Low stomach acid is a real condition, more common in older adults and in people on long-term PPI therapy, and it can cause digestive symptoms including some that overlap with reflux. However, it is a less common cause of reflux symptoms overall than a weak LES with normal or excess acid, so this approach isn't appropriate for most people with typical GERD.

Why this needs real caution

Taking betaine HCl when your reflux is actually caused by normal or excess acid (the much more common scenario) is likely to significantly worsen symptoms and could cause real esophageal irritation. This isn't a supplement to try casually or without a clearer sense of your underlying acid levels.

The responsible approach

This is a supplement that should only be considered under the guidance of a knowledgeable practitioner, ideally after some indication (through testing or a detailed symptom history) that low acid, rather than high acid, is genuinely part of the picture — not a first thing to try based on general reflux symptoms alone.
